Partner in Your Care is an established and successful provider based in Sydney, ACT and South Australia whose one main objective is to enhance their Participants’ quality of life and enable them to continue to live in their home by providing opportunities for choice and inclusion. In consultation with Participants and family carers, the Partner in Your Care team identifies the optimum Individual Living Options for each Participant. Implementation of these options often occurs in stages and the package includes a primary support approach and supplementary supports as needed. Once established, the living arrangements are closely monitored and continually fine-tuned.

Partner In Your Care also offers a range of group accommodation options including apartments, villas, and houses each within a safe, secure and welcoming environment. These are specially tailored for low, standard, or complex support needs as required. They provide for all needs, for just a few hours or for full time care.

Through their Supported Accommodation Services, Partner in Your Care helps Participants to learn and develop everyday skills, be part of a community, live independently with others in a group setting and to support Participants with community access (with specialised transport) such as shopping and medical appointments. Participants are assisted with household tasks, cooking, cleaning, personal care, shopping, money management and budgeting, and accessing Government services such as Centrelink.

Partner in Your Care offers a respite service that enables family carers time to attend to their own needs. The team has a 24 hour on-call service that can be accessed in case of emergency. They offer full in-home support for carers, access to outside services such as swimming or sports clubs, disability service providers, assistance with shopping or banking, or access to community activities.

Partner In Your Care work collaboratively with participants stakeholders to provide consistent tailor-based supports for all participants. Partner In Your Care provide a range of experienced Qualified support staff who have access to tailor based training to assist participants with their support needs and to work hand in hand to support them with their NDIS goals.

Partner in Your Care provides mental health services that support people to improve their wellbeing and to achieve their goals. Issues such as loneliness, mobility issues, transport and isolation can be alleviated as they discuss and plan services that will be best suited to each Participant.
